Immunization Requirements
School Immunization Requirements
For attendance in ALL grades, children need the following properly-spaced immunizations for attendance:
- Four doses of tetanus, diphtheria, and acellular pertussis *(1 dose on or after the 4th birthday)
- Four doses of polio (4th dose on or after 4th birthday and at least six months after the previous dose given)
- Two doses of measles, mumps, and rubella**
- Three doses of hepatitis B
- Two doses of varicella (chickenpox) or evidence of immunity
* Usually given as DTP or DTaP or DT or Td ** Usually given as MMR
Students in grades 7-12 require these ADDITIONAL immunizations for attendance:
- One dose of tetanus, diphtheria, acellular pertussis (TDaP) - Given at age 11
- Two doses of meningococcal (MCV) conjugate ACWY vaccine (Menactra®, Menveo®, and MenQuadfi®). The first dose is given at 11-12 years of age. The second dose is required at age 16 or entry into 12th grade. If the first dose was given at age 16 or older, only one dose is required.

Exemptions to the school laws for immunizations are:
- Medical reasons
- Religious beliefs
- Philosophical/strong moral or ethical conviction
If your child is exempt from immunizations, he or she may be removed from school during an outbreak.
