Sports Physicals
Student-athletes entering grades 7-12 will need a Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) form handed into the athletic trainers prior to in-season practices, scrimmages, and gameplay. The CIPPE may not be authorized earlier than May 1, 2024, and shall be effective - regardless of when performed - until the conclusion of the 2025 spring sports season.
Registration Deadlines
The deadlines to submit sports physical paperwork for the 2024-2025 school year:
Football - July 29
All Other Sports - August 5
Winter Sports - November 8
Spring Sports - February 24

Activity Fees
Activity fees can be paid online or by submitting payment in cash or by check. For any high school students participating in an activity, the fee will be $50 for a sport and $35 for a club. The maximum PER STUDENT fee is $50 while the maximum PER FAMILY fee is $150 per school year.

Where to Get a Sports Physical
There are other options for students who need sports physicals and are too close to their yearly wellness visit to have it covered by insurance:
Orthopedic Associates of Lancaster offers sports physicals done by Dr. Eck., the group's sports medicine doctor. Physicals are $15 due at the time of appointment. No insurance is/will be charged for them. Please call 717 299-4871 to schedule a sports physical with Dr. Eck. Appointments can be scheduled between 7:30 a.m. and 4:45 p.m. Dr. Eck works out of the Lancaster, Spooky Nook, and Lebanon offices. See locations
Providers at urgent care facilities usually offer sports physicals. We recommend calling the desired urgent care to check payment options
School-sponsored sports physicals take place on campus shortly after the end of the school year. We will announce the date and post a sign-up in March.
Summer sports physical clinics (OAL) will be announced closer to the end of the school year. We will post information here.
Frequently Asked Questions About Sports Physicals
- Why do I need a sports physical? All student-athletes are required by the PIAA (Pennsylvania Interscholastic Athletic Association) to have a physical examination before participation in any school-sponsored athletic activity.
- When and where are sports physicals? Sports physicals are typically offered at the Lampeter-Strasburg High School on the first Saturday in June. This is the ONLY time that sports physicals will be offered at L-S for the forthcoming school year.
- How much does an L-S-sponsored sports physical cost? $20.
- How long is a sports physical valid? Physicals for the forthcoming school year must take place on or after June 1. Physicals completed before June 1 will not be accepted. A sports physical is valid from when the doctor signs it until May 31.
- Can I go to my own doctor for the sports physical? Yes.
- What is a CIPPE form? CIPPE stands for Comprehensive Initial Pre-participation Physical Examination.
- Can I use a different form? No. The PIAA requires all student-athletes to use the CIPPE form.
- Will I need another sports physical for a winter or spring sport? No. Once a completed CIPPE form (sections 1-6) is on file, a second physical is unnecessary. However, a re-certification form will be required. 9. What is a re-certification? Re-certification is required for all student-athletes participating in subsequent sports in the same school year or if the initial sports physical took place earlier than 6 weeks before the start of the winter or spring sports season. CIPPE Section 7 Parent/Guardian Re-certification Form This is required for ALL athletes whose sports physical took place on or after June 1 but earlier than six weeks prior to the start of the winter or spring sports season. CIPPE Section 8 Doctor Re-certification Form This is ONLY required for athletes who see a healthcare provider for an injury or illness occurring after the sports physical but before the start of the subsequent sports season.
